Position Title: Transitions Coordinator (Case Manager)

Full time, 37.5 hours per week
Salary and Benefits:
$33,150. Vacation, holidays, sick days and personal days, medical insurance
(50%) according to the guidelines in Vita’s Policy Manual.
Background: Vita has created a position
called Transitions Coordinator.
The Transitions Coordinator will
provide intensive case management services to include on-going structured and
documented individual educational, economic, personal, and career counseling for
adult students. Services may include academic and aptitude assessments
with interpretations; short and long-term goal setting using the SMART paradigm; career exploration; values education; access to basic technology
education; job development: on-going personal development, life-long learning
training, portfolio development; pre employment assessment; and transitional and
support needs assessment. Activities will support identifying personal strengths
and developmental needs, strategizing and identifying resources, addressing
personal issues that impede program participation and successful program
completion and developing strategies to address those issues, and follow-up
tracking services for up to one year after the adult leaves the program.
Summary
of job:
The Transitions
Coordinator provides case management to selected adult learners to track their
progress in the Vita program. The Transitions Coordinator will interact with staff and community
agencies to help students realize their education, career and academic goals.
Both day and evening hours are
required. Travel is necessary for coordination and meeting with students
(mileage is reimbursed).
Responsibilities:
- Participate in Case
Management Supervisory Team to develop policy and a system for case
management referrals.
- Communicate with teachers
to identify potential students.
- Interview and assess
potential students
- Provide supportive
counseling and educational/vocational guidance
- Develop and implement goal
setting activities in individual and class situations.
- Monitor student progress
through regular contact with staff, student and participating agencies
- Maintain student files
- Track students one year
after completion of the Vita program
- Participate in local and
state initiatives to learn and share resources
- Provide documentation to
meet performance standards
- Participation in state
training and staff meetings
- Execution of other
responsibilities as assigned
Qualifications:

Minimum of bachelor’s
degree in education or related field
